Baptismal Registration Form

This registration form is only for the Sacrament of Baptism.

If your child is between 2nd and 6th grade,you can also register to the First Reconciliation and Communion program.
Candidates in 7th grade and above will be prepared through the Confirmation program. They will receive formation on all three sacraments, Baptism, Confirmation, and Communion. Please visit our Faith Formation page for program descriptions:
