This form is primarily for three things:
1) Submit 2nd registration deposit ($375 per person)
2) Submit Housing/Roommate requests
3) Confirm contact information & travel details

Roommate/Housing Requests
Housing is assigned 4 people per room, each with 2 queen beds. We will do our best to accommodate requests, but cannot make guarantees. Adults will be roomed with adults of the same sex (except in the case of a married couple). Youth will be assigned to rooms of the same sex. Youth may sleep in the same room as a parent/guardian, but no youth may stay in a room with an adult who is not his/her parent/guardian. Additional charges may be applied if your request will result in additional charges to the Diocese.
Note on Phone and Email:
Cell phones are not required, nor will they be banned from the event, although appropriate use will be enforced.
They are ubiquitous and are very useful in the case of emergency. If your teen has a cell phone, please input their cell phone number (not mom's or dad's) so that we can quickly communicate information to them during the event, especially in cases of emergency. The same goes with email. If the youth does not have a cell phone, then you may leave it blank.
