Wedding Registration Form

Please fill out this form in order to schedule your wedding at the National Shrine Grotto. There will be a $500 fee (plus any additional needs described below). A $250 non-refundable deposit is due upon registration. The remaining balance is due 60 days prior to the wedding ceremony date.
