This form is to be used by Boards, Ministry Groups, ECC, Youth, etc. within St. Matthew Lutheran Church & ECC that are wanting to have a fundraiser, request donations from congregation or have a special door offering during a church service. This form is to be submitted to the Board of Deacons a month before the event date for approval.
