Baptism Record Form

One of the prerequisites for Sacraments of First Communion and Confirmation is Christian Baptism. It is the responsibility of the parents to provide Holy Redeemer with a copy of their child's Baptismal certificate. This Baptismal certificate must be issued from the parish the child was baptized at NO MORE THAN 6 MONTHS PRIOR TO THEIR FIRST COMMUNION/CONFIRMATION DAY. It is absolutely necessary that you provide us with this proof of Baptism.
